Glassesusa Extended Warranty

I found most of the warranty's from eyeglasses companies are not very good and only cover the partial cost of replacing the glasses.

I see that Glassesusa.com offers an extended warranty over the 1 year manufacturers warranty which includes prescription changes and accidental damage. I was quoted $12 which seems like areally good price. Has anyone purchase the extended warranty and used it? Did you have any problems?
